After completing an enrollment packet, the following documentation is required by Kentucky Law.

A Kentucky Immunization Certificate. These will be available through your health care provider showing all age appropriate vaccines. NEW! Kentucky Immunization requirements for school. Original legal certificate of birth (KY) or other reliable proof of birth required by State Law 158.032. ( OH birth form)
School Medical Examination Form completed by family physician; current within 12 months of enrollment. If medication is required at school a Kentucky Medication Form will also be required and to be filled out by your health care provider.
First time enrollees to Kentucky public schools also require a completed Ky Eye Examination Form performed by an Optometrist or Ophthalmologist. Official custody documentation (if applicable) Additional forms throughout the year as required by district, state and federal policies and procedures. |
